Cheek fillers to fill out his cheekbones. At his age, skin thins out and stuff sags, including fake stuff.

 

As far as nodding off, I sometimes do that watching TV, for maybe a few minutes. That's what happens with us old people. Sitting still in a mostly quiet place. Plus him staying up late isn't helping him. I bet Biden is getting pretty good sleep these days, isn't up all night ranting on social media.
