That guy is going to need wound care for that!  Looks like a huge boil.  A buddy in Basic had one on his leg, eventually had to go to the hospital because he kept squeezing it and it got infected. Left him with a half inch deep inch or so in diameter hole on his leg. That's about two hours from me, but there are several more much closer. A few years ago one preacher was arrested within 30 minutes of me for violating a law that restricts the number of poisonous snakes you're allowed to have.
